Community and Supportive Services
The Community and Supportive Services (CSS) Program Case Management program provides referrals and assistance to the former residents who lived at Central Park Village and current ENCORE residents as part of the Encore Choice Neighborhood Initiative. The program is comprised of three phases:
- Family Needs Assessments/Development of Case Plans
- Referral and Service Delivery
- Monitoring and Re-Assessments.
This case management service offers specific programs that are designed, modified, and tailored to fit the resident's individual lifestyle. The program has now expanded to include the new West River redevelopment initiative, serving former residents of North Boulevard Homes and Mary McLeod Bethune Homes.
